Before you arrive
Your first visit
When you come for your appointment, please remember to bring the following:
- Driver's licence or a valid ID
- Insurance information
- Referral letter, if one is required
- Reports, X-rays, MRIs, CT scans and any other relevant information
- A list of your current medications
Appointments
Cancelling an appointment
Telephone the office during business hours and allow at least 24 hours' notice, so that we can offer your appointment time to patients on our waiting list.
We recognise that your time is valuable and we make every effort to run on time. Occasionally emergencies arise, or a patient requires a little more time, and this may cause scheduling delays beyond our control. We apologise if we keep you waiting.

Travelling for your surgery
We offer concierge care for patients travelling from out of region and from abroad. Dr. Denard and Dr. Phillips have treated patients from all over the country as well as Canada. Our team is happy to assist in coordinating your travel so that you and your family have a smooth experience.
To become a patient or get a second opinion, call us on 541-608-2595 or contact us to set up an initial video consultation with either Dr. Denard or Dr. Phillips. Most telehealth or video consultations are covered by insurance.
Insurance and self-pay
Oregon Shoulder Institute accepts all major insurance providers. Please call our office with any insurance-related questions.
Oregon Shoulder Institute is also committed to honest, transparent pricing. Cash prices are for self-paying patients only. Patient costs are all-inclusive, covering the facility, surgeon and anaesthesia fees. There are no hidden costs.
Telehealth appointments
A virtual visit, or telehealth visit, allows an evaluation through video conferencing. This lets you connect with a physician and obtain an evaluation without coming into the office. A virtual visit is particularly useful for obtaining an opinion from a provider outside your normal area, or for patients who would otherwise travel a long way to see a doctor.
