Can You Play Pickleball After Shoulder Surgery?

Pickleball has become one of the fastest-growing sports in America — and for good reason. It's social, accessible, and lower-impact than tennis. Over 90% of pickleball players are aged 50 or older, which also happens to be the group most likely to develop the shoulder conditions that sometimes require surgery.

If you're a pickleball player who has been told you need shoulder surgery — whether a rotator cuff repair, arthroscopy, or shoulder replacement — you're probably asking yourself one thing: Will I ever play again?

We designed a study specifically to answer that question. It's the first of its kind to look at pickleball players exclusively, and the results are encouraging.

Why Pickleball Deserves Its Own Research

Most return-to-sport studies after shoulder surgery group all racket sports together or focus on tennis. But pickleball is not tennis. It features shorter rallies, an underhand serve, and far less overhead striking — a fundamentally different biomechanical profile that places lower demand on the shoulder. Whether that translates into better recovery outcomes compared to tennis players hadn't been studied.

We also knew from clinical experience that our pickleball patients were highly motivated to get back on the court. For most of them, pickleball isn't just exercise — it's a daily routine, a social community, and a major part of their quality of life. Getting real data to help set realistic expectations matters.

How the Study Was Done

We reviewed outcomes for 23 patients at the Oregon Shoulder Institute who identified pickleball as their primary sport before undergoing shoulder surgery between 2012 and 2022. The group included 16 patients who had shoulder arthroscopy (primarily rotator cuff repair), 3 who had anatomic total shoulder arthroplasty (aTSA), and 4 who had reverse total shoulder arthroplasty (rTSA).

All patients had at least two years of follow-up, with an average of 44 months. The average age was 66 years. Patients completed a detailed questionnaire about return to play, intensity, frequency, and pain, alongside objective range-of-motion measurements and validated shoulder outcome scores.

The Results: Everyone Got Back on the Court

The headline finding is clear: every single patient returned to playing pickleball by the end of the study period.

100% of all patients returned to pickleball at final follow-up. 78% returned within the first year of surgery. 87% were able to return to the same level of participation as before their procedure. And 57% reported no pain at all after playing pickleball postoperatively — compared to zero before surgery.

Patient satisfaction was high across both groups. Among arthroscopy patients, 56% were completely satisfied with their postoperative performance and 31% mostly satisfied. Among arthroplasty patients, 43% were completely satisfied and 57% mostly satisfied.

Post-play pain also improved dramatically. Before surgery, the large majority of patients experienced lingering discomfort for 15 minutes or longer after a session. After surgery, more than half reported no pain at all.

Arthroscopy vs. Replacement: The Timeline Differs

One of the more nuanced findings involves the difference in recovery timelines between procedure types.

Arthroscopy patients returned faster — 88% were back on the court within 12 months, with many returning between 3 and 6 months. This aligns with what we see clinically: arthroscopic rotator cuff repair involves no joint reconstruction, and the rehabilitation pathway, while rigorous, generally allows return to lower-demand sports within 6 to 9 months.

Arthroplasty patients took longer — only 57% had returned within 12 months, though 100% ultimately returned. This is expected. Shoulder replacement is a larger procedure with a longer recovery curve. The subscapularis tendon must heal, soft tissues need time to adapt around the new implant, and loading is introduced progressively. But the payoff is significant: once recovered, arthroplasty patients experienced dramatic improvements in range of motion, pain scores, and overall shoulder function.

The key takeaway is not that replacement patients do worse — they don't. They simply need more patience in the first year. Planning for a 12-to-18-month window before returning to full-intensity pickleball is reasonable and appropriate.

Why Pickleball Outcomes May Be Better Than Tennis

Our 100% return rate — with 78% within the first year — compares favorably to what has been reported for tennis players after shoulder surgery. Studies on tennis specifically show return rates in the 65–78% range after shoulder arthroplasty, with most requiring 7–10 months.

The likely explanation comes down to sport demand. Tennis involves frequent, powerful overhead serves and high-velocity groundstrokes that place extreme rotational load on the shoulder. Pickleball's shorter court, underhand serve, and lower-velocity exchanges create a more forgiving biomechanical environment for a recovering shoulder.

This doesn't mean pickleball is risk-free. Falls on the court are a real concern in older players, and repetitive reaching and lateral movement do stress the rotator cuff over time. But from a return-to-play standpoint, pickleball's lower mechanical demands work in patients' favor.

What This Means If You're Considering Surgery

If you're on the fence about shoulder surgery because you're worried about losing pickleball, here's what this data suggests.

If you need arthroscopic rotator cuff repair, expect a return to pickleball in roughly 6–12 months. Most patients in our series were back within that window, and the majority returned to their prior level of play.

If you need shoulder replacement, plan for a longer runway — 12 or more months before you're back to regular play. But your odds of returning are very high, and your shoulder function will likely be meaningfully better than it was before surgery.

It's also worth noting that pickleball's benefits go well beyond physical fitness. Research consistently shows improvements in mental well-being, social engagement, and life satisfaction among older pickleball players. Getting back to the sport is about quality of life — and that makes it a meaningful surgical goal, not a secondary concern.

The Bottom Line

This is the first study to look specifically at pickleball players following shoulder surgery, and the results are about as encouraging as they could be. Every patient returned to the sport. The vast majority came back at the same level they played before. Pain after playing decreased sharply. And satisfaction was high across both arthroscopy and arthroplasty groups.

If shoulder pain is keeping you off the pickleball court and surgery has been recommended, don't let fear of losing the sport drive your decision. The evidence suggests it won't be taken from you — and in many cases, you'll come back playing better than before surgery was holding you back.

The right surgical choice depends on your specific diagnosis, anatomy, and activity goals. The best next step is a thorough evaluation to understand your options and what recovery looks like for your situation.
